Forms Builder

The Forms Builder facilitates the creation of user-friendly web forms by chamber administrators. These forms can be readily accessed by both chamber members and the general public, making them ideal for collecting information through scholarship applications, public opinion polls, etc.


The Forms Builder is a convenient tool that allows your organization to create simple web forms for members or the general public to fill out. These forms can be used for various purposes such as scholarship applications, public polls, or any other needs you may have. Each time a user submits the form, its contents are sent to a designated email address.


To access the Forms Builder, follow these steps:

1. Go to the Administrative Functions section of the main menu.

2. Select "Form Builder" to enter the Forms Builder interface.

3. In the Forms Builder menu, you can add, modify, or delete forms.


Adding a New Form:

1. Click on "Add New Form" to start creating a new form.

2. You will be guided through a series of pages to configure your form.


General Info:

- Enter a name and description for the form.

- Specify an email address where the form contents will be sent upon submission.

- Check the box to prompt for Name/Email if you want to include fields for first name, last name, and email.

Questions:

- To add a question, click on "Add New Question" and choose the question type (Multiple Choice, Yes/No, Text Field, Multiline Text).

- Enter the question verbiage and, if applicable, set the options for multiple-choice questions.

- For multiple-choice questions, you can customize the choices on a separate page.

- Use the "delete" or "change" links next to a question to delete or modify it.

- Reorder questions using the "move up" and "move down" links.


Publishing:

- Once you've created the form, you will find its URL listed in the Forms Builder admin area.

- Copy and paste this URL into emails, web pages, or any other platform where you want to publish the form.

Track Responses to Member Profile:

- There is a configuration option to track members' responses in their individual member profiles.

- By enabling this option, you can link form submissions to specific member profiles for easy tracking and analysis.

Utilize the Forms Builder to create customized web forms for various purposes.
